If you're missing multiple teeth, you've probably encountered two options that both involve titanium implants anchored in the jawbone: traditional dental implants (placed individually for each missing tooth or small group of teeth) and All-on-4 (a full-arch solution using just four strategically placed implants). Both are permanent. Both look natural. Both restore chewing function. So how do you know which one is right for you?
The answer depends primarily on how many teeth you're missing and the condition of your remaining teeth.
## Individual Dental Implants: The Right Choice When
Individual dental implants — one implant per tooth, or two implants supporting a bridge for two to three adjacent missing teeth — are the ideal solution when:
You're missing one to several teeth, but most of your natural teeth are healthy. If you have good dental health overall but are missing one, two, or three teeth scattered across your mouth, individual implants replace only what's missing while leaving healthy natural teeth untouched.
You want the most lifelike restoration. A single implant-supported crown is the closest thing dentistry has to a natural tooth. The crown is custom-made to match your surrounding teeth precisely, and because it's anchored by its own implant, it functions completely independently.
You have sufficient bone in the specific areas where teeth are missing. Individual implants require adequate bone volume at each implant site. If bone loss has occurred at specific locations, bone grafting at those sites may be needed before implant placement.
## All-on-4: The Right Choice When
All-on-4 uses four implants per arch — two straight in the front and two angled toward the back — to support a full arch of fixed teeth. It's designed specifically for patients who are:
Missing all or most teeth in an arch. If you're missing an entire arch of teeth, or most of your remaining teeth are failing and require extraction, All-on-4 delivers a complete, fixed set of teeth with just four implants and one surgical appointment.
Currently wearing a full denture and want a permanent upgrade. All-on-4 converts a removable denture situation into a fixed, implant-supported arch. The result is dramatically more stable, comfortable, and functional than any denture.
Dealing with bone loss in the back of the jaw. Because the posterior implants are angled, they access denser bone in the front of the jaw — which means many patients with some bone loss in the rear of the arch can still receive All-on-4 without bone grafting. This is one of its greatest advantages over placing traditional implants in the back of the jaw, where bone is often deficient after tooth loss.
Facing the cost of restoring many individual teeth. If you're missing eight, ten, or twelve teeth, replacing each with an individual implant would require eight to twelve implants, eight to twelve surgeries, and eight to twelve crowns. All-on-4 replaces a full arch with four implants and one fixed prosthesis — a more efficient and cost-effective approach for extensive tooth loss.
## When Both Approaches Are Used Together
Some patients need a combination. For example: individual implants in the upper arch (where only a few teeth are missing) and All-on-4 in the lower arch (where all teeth are missing or failing). Treatment is always customized to your specific anatomy and goals.
## What Does the Evaluation Process Look Like?
Whether you're considering individual implants or All-on-4, the starting point is a comprehensive consultation that includes:
- **Panoramic X-ray or cone beam CT scan** (3D imaging) — to assess bone volume and density at all potential implant sites
- **Medical and dental history review** — to identify any health factors that affect implant candidacy
- **Discussion of your goals** — how many teeth are missing, what your experience with any current prosthetics has been, and what outcome you're hoping for

## What About Bone Loss? Does It Rule Out Either Option?
Bone loss is a common concern for patients who've been missing teeth for some time. Here's how it affects each approach:
For individual implants, bone loss at the specific missing tooth site may require bone grafting before the implant can be placed. This adds time and cost but is routinely performed at our office under IV sedation.
For All-on-4, the technique was specifically designed for patients with bone loss in the posterior (back) jaw. The angled posterior implants engage denser bone in the front of the jaw, bypassing areas of deficiency. Many patients who have been told they're "not a candidate" for individual implants due to bone loss are excellent candidates for All-on-4.
This is one of the most important reasons why a consultation and cone beam CT scan — not just a conversation — are essential. The imaging tells us definitively what's possible and what the optimal path is.
## What About the Timeline? How Long Does Each Approach Take?
Individual implants (single tooth):
- Implant placement: 1 procedure
- Healing: 3–6 months
- Crown placement: 1 appointment with your restorative dentist
- Total timeline: 4–8 months
All-on-4 full arch:
- Extraction of failing teeth + implant placement + temporary teeth: same day
- Healing: 3–6 months
- Final permanent arch: 1–2 appointments
- Total timeline: 4–8 months (but you have functional teeth throughout)
Both options have similar total timelines. All-on-4 provides the unique advantage of functional temporary teeth immediately from day one.
