What to bring to your first visit

A little preparation makes your first appointment faster and smoother. Completing the registration form below ahead of time means less paperwork in the waiting room and more time with your surgeon. Please plan to bring the following with you:

• Photo ID and your dental and medical insurance cards
• A referral or recent X-rays from your dentist, if you have them
• A complete list of medications, vitamins, and supplements you take
• Your medical history, including allergies and any prior surgeries
• The name and location of your preferred pharmacy for prescriptions
• If the patient is a minor, a parent or legal guardian must attend

What to expect

Your first visit is usually a consultation. One of our board-certified oral surgeons will review your history, take any imaging that is needed, examine the area of concern, and explain your treatment options in plain language. We will walk through anesthesia choices, recovery, and cost - and answer every question you have before anything is scheduled. Most consultations take about 45 to 60 minutes, and in many cases minor procedures can be completed the same day.
