Oral Surgeons of Indiana (“we”, “us”, “our”) operates the website at https://www.oralsurgeonsin.com. This policy applies only to information collected by this website and does not cover protected health information (PHI) maintained in our patient records, which is governed separately by HIPAA and our Notice of Privacy Practices provided at the office.

Your Choices

Data Retention
Contact-form submissions are retained in our office email for as long as needed to respond to your inquiry and meet our recordkeeping obligations. Aggregate analytics data is retained according to Google’s default retention period.
